PLEASE READ FIRST: Here are a couple of important instructions for downloading the editable pdf forms.

1) Open Adobe Reader before you download any form – this will help you “see” the form once you download

2) When you click the download link you will get a page that reads “Please wait…” (see attached image) this is expected.

3) Click the download icon on the “Please wait…” page. After you do that your form will appear as expected in Adobe Reader.

4) Save all your forms in a “Training Library” folder so you can quickly reference when needed and they are all in one spot. Of course, you will always be able to access via your PPO Insider Library too. 🙂
